Request a Vote by Mail Ballot


Florida Law requires that if the ballot is requested to be mailed to an address other than the elector’s address on file in the Florida Voter Registration System, the request must be made in writing, signed by the elector, and delivered to the Supervisor of Elections office. (F.S.101.62)

You may request a vote by mail ballot in writing using one of these forms:  English  Spanish

Voted mail ballots must be received by the Supervisor of Elections' office no later than 7 p.m. on Election Day. If you are returning your ballot by USPS, allow 3-5 delivery days for return. A voted mail ballot cannot be accepted at a polling place.
